BREAST RECONSTRUCTION AFTER MASTECTOMY

Breast reconstruction is a surgical procedure  to rebuild the shape of the breast after a mastectomy (breast removal due to cancer). It helps restore the breast’s appearance and supports emotional well-being after cancer treatment.

When Can Breast Reconstruction Be Done?

Immediate reconstruction: Performed during the same surgery as the mastectomy.

Delayed reconstruction: Done months or even years later, once cancer treatment is complete.

Types of Breast Reconstruction

1. Implant-Based Reconstruction

Uses saline or silicone breast implants to restore breast shape. Implants can be placed:

Under or over the chest muscle, sometimes supported by an acellular dermal matrix (ADM) for added coverage.

As a one-stage (direct-to-implant) procedure or

In two stages using a tissue expander, which is gradually filled over time before placing the final implant (3–6 months post-mastectomy).

2. Autologous Tissue (Flap) Reconstruction

Involves using your own skin, fat, and/or muscle from another body area to recreate the breast.

Common donor sites include the:

  • Lower abdomen (DIEP or TRAM flap)
  • Back (Latissimus Dorsi flap)
  • Thigh or buttocks (PAP, TUG, SGAP, IGAP flaps)

Flaps can be:

  • Pedicled flaps: Moved to the chest with their original blood supply intact.
  • Free flaps: Fully detached and reconnected using microsurgery.

Matching the Other Breast

To achieve the best symmetry possible, your surgeon may recommend procedures on the opposite breast, such as a:

  • Breast lift (mastopexy)
  • Breast reduction
  • Breast augmentation

Final Touches: Nipple and Areola Reconstruction

If the nipple and areola were removed, they can be recreated later using surgical techniques or 3D tattooing.

Choosing Breast Reconstruction vs. Prosthesis

Breast reconstruction is a personal decision. Some patients choose not to undergo reconstruction and instead wear a breast prosthesis, a silicone form placed in a bra to mimic natural breast shape under clothing.

Depending on your breast shape, your surgeon may also recommend surgery to the normal breast to match the size and shape of the reconstructed side – either a breast reduction, breast lift or breast augmentation. During the final stage of breast reconstruction, a nipple and areola is re-created on the reconstructed breast, if these were not preserved during the mastectomy.
